The Clinical Experience of Atypical Superior Segmental Optic Hypoplasia Associated with Cystoid Macular Edema

Abstract
Purpose: To Report a case with atypical superior segmental optic hypoplasia. The risk factors of superior segmental optic hypoplasia(a specific form of congenital disc hypoplasia) are low birth weight, prematurity, poor maternal diabetic control. Two cases were reported in Korea. Normal visual acuity, thinning of superior peripapillary nerve fiber layer and corresponding inferior visual field defects are the characteristics of this disorder. Relative superior entrance of central retinal artery, pallor of the superior optic neural rim and a superior scleral halo can be observed. However, we experienced one case of atypical SSOH with cystoid macular edema (CME) and without inferior altitudinal defect in visual field.

Methods: We examined a 23?year?old male who has complained of low vision in both eyes since elemenatary school. Visual acuity, manual refraction, anterior segment examination, fundus examination, visual field, fluoresein angiography (FAG) and optical coherence tomography (OCT) were performed.

Results: BCVA of the right eye was 20/30. BCVA of the left eye was 20/50. He had no history of maternal diabetes. Fundus examination of both eyes revealed pallor of the superior optic neural rim, relative superior entrance of central retinal artery and a superior scleral halo. Thinning of the superior peripapillary nerve fiber layer and corresponding inferior visual field defects were not evident. But, OCT revealed thinning of superotemporal peripapillary nerve fiber layer on the right eye and thinning of superonasal peripapillary nerve fiber layer on the left eye definitely. CME was shown on fundus examination and OCT of both eyes. But, fluorescein leakage was not shown.
